This only happens in the 64-bit version, the 32-bit version works fine.
1. Press Ctrl+M to bring out the Multi-Rename Tool
2. Click one of the "+", "#" and "plugin" buttons to show the context menu or plugin property dialog box
3. Press Esc to dismiss the context menu or plugin property dialog box
4. Keep the Multi-Rename Tool open and press Alt+Tab to switch to another window
5. Press Alt+Tab again to switch back to Total Commander
6. The file name combo box disappears, there is only a white rectangle in its place
7. If you move the mouse over the rectangle, the combo box shows again
